MSW School Life

Interscholastic Athletics - Grades 4 thru 8

Softball, Baseball, Track and Field, Cheerleading, Flag football, Cross Country, Basketball, Volleyball, and Soccer

The Mary, Seat of Wisdom Athletic Committee, comprised of parents and other adult volunteers, coordinates an interscholastic athletic program supplemental to the school’s educational curriculum.

All children are given the opportunity to participate in any athletic program provided they follow the published guidelines. The athletic program is integrated into the Catholic Christian tradition of the school. The program develops the total student by integrating good sportsmanship, teamwork, and leadership. Parental/guardian participation is expected to aid in achieving this goal.

Beginning in 4th grade, the following sports are offered: boys and girls basketball, boys and girls cross-country, boys and girls track, and boys and girls volleyball. Co-ed soccer is offered beginning in fifth grade. Boys baseball and girls softball is offered to 7th and 8th grade students.

The School’s Extended Day Program hosts various intramural athletic activities which are generally offered immediately after school. For more information, visit the Extended Day webpage.

The following are the required forms for all MSW athletes to be submitted to the school prior to the first practice of all sports. Failure to complete the following forms prior to the first practice will exclude the student from the sport until they are submitted.

- IHSA/IESA Pre-Participation Physical Examination Form submitted to the school office prior to the first practice. Required by Illinois Department of Public Health (IDPH), Illinois State Board of Education (ISBE) and Illinois Elementary School Association (IESA),

ISHA/IESA Concussion Information Sheet to be read by the parents and student before signing the acknowledgement form below. Required by Illinois Department of Public Health (IDPH), Illinois State Board of Education (ISBE), Illinois Elementary School Association (IESA), and Illinois Youth Sports Concussion Safety Act

- Parent/Student Acknowledgement and Consent Form must be signed after reading the above concussion information sheet by the parent and student and submitted to the school office prior to first sport practice. Required by Illinois Department of Public Health (IDPH), Illinois State Board of Education (ISBE), Illinois Elementary School Association (IESA), and Illinois Youth Sports Concussion Safety Act

IHSA IESA Pre-Participation Physical Examination Form - ALL students participating in MSW School sports must have a physical examination form on file with the school office. This will be mandatory. Everyone registered for a school sport MUST have a physical examination form submitted by the first game in order to be eligible to play.

Students should bring their sports uniforms with them to school.
PMI is currently accepting online orders only.
Orders must be submitted prior to OR on picture day to avoid a service fee.
