Enrolling a New Student
MSW has an on-going registration process. Families may apply for admission at any point during the school year by filling out the New Student Application and submitting the designated fees, birth certificate, and sacramental records. For those joining us for grades 1-8, a RECORDS RELEASE FORM will need to be completed as well.
NEW STUDENT APPLICATION (PAPER FORM ONLY)
The following items are required when registering.
- Application form
- Original Birth Certificate – which will be copied and immediately returned to you
- Copy of baptismal certificate (if applicable)
- Application fee – $100 cash or check per family (non-refundable)
- Copies of any IEP, ICEP, 504 Plan or other academic, health, or social-emotional support currently needed.
- MSW Medical Handbook for all required medical forms by grade. mswschool.org then News, Events and Information tab, then Handbook
NOTE: The State of Illinois mandates that incoming Kindergarteners MUST be 5 by September 1st of the year they enter Kindergarten. Please be aware of this when filing your child's Application.

Re-registration for Current MSW Students
MSW Pre-K through 7th grade students will automatically be re-registered for the next school year. Please contact Joan Mattingly at the school office if you will not be returning to MSW.

Re-registration Fee
No need to send payment! All families will be billed a one-time $100 re-registration fee through FACTS Incidental Expenses. PLEASE NOTE: The $100 re-registration fee is per FAMILY, NOT per STUDENT. If you are re-registering a student and also enrolling a new student, your FACTS account will be billed a one-time $100 registration fee. Please contact Ms. Rose Reaves with all registration fees and tuition questions at RREAVES@MSWPARISH.ORG or 847-825-1522.
Please DO NOT USE the New Student Paper Application form for re-registering students or siblings.
Age Requirements
Preschool – A child entering preschool must be three and/or four years of age on or before September 1st of that year.
Kindergarten – A child entering kindergarten must be 5 years of age on or before September 1st of that year.

- Meet with a member of our parish staff to connect and explore ways to engage in parish life
- Complete the Parishioner Registration Form after your meeting
After completing this process, your tuition will reflect the Parishioner rate
